The global aesthetics market has quietly grown from a niche corner of medicine into a mainstream, multibillion-dollar industry. Surgical procedures—facelifts, liposuction, body contouring—remain high-ticket but are limited by recovery time, risk, and patient tolerance. Non-surgical aesthetics, by contrast, is where the real growth has occurred: repeatable treatments, lower risk, and far broader consumer adoption.

I’ve spent the past two decades in New York—arguably the richest and most competitive aesthetics market in the world—focused exclusively on the non-surgical side. We were likely among the first practices to go 100% non-surgical, recognizing early that many patients wanted results without surgery, rather than being a plastic surgery practice that simply added Botox and fillers.

Today, forecasts suggest the non-surgical market alone could more than double—from roughly $19.5 billion in 2025 to over $40 billion by 2031, with global estimates for injectables and energy-based devices exceeding $80 billion.

2001: The Beginning — Just Botox and Fillers

Twenty years ago, non-surgical aesthetics was simple. There were essentially two tools: Botox and dermal fillers. They offered predictable, short-term results, but there was no real technology behind rejuvenation—no way to address the structural aging of skin, only temporary fixes for wrinkles or volume loss. That changed with the first meaningful device breakthrough.

Thermage: Collagen Without Burning the Skin

In 2001, the first true aesthetic “device” entered the market. Radiofrequency (RF) energy was introduced as a way to heat deeper layers of skin without damaging the surface. For the layperson, the analogy is simple: it’s the frustration of trying to brown chicken skin in a microwave. Thermage figured out how to harness that energy and direct it precisely to the collagen layer, tightening skin and stimulating collagen production without burning the surface—and patented the approach.

Our medical director, Dr. Lisa A. Zdinak, along with Danielle Braz, was directly involved in testing and validating the technology. At the time, we shared space with one of New York’s most respected plastic surgeons. His verdict was blunt: “It’s a toy.” Within a few years, that same surgeon was asking us to tighten and build collagen before performing facelifts.

2009: Exilis and the Next Wave

Another inflection point came in 2009 with the introduction of Exilis by BTL. Dr. Zdinak was lecturing on Thermage at the Monte Carlo Anti-Aging Congress, presenting one of its most meaningful upgrades. At that conference, after meeting BTL’s president—now best known for Emsculpt—we were tasked with bringing the first Exilis system to the United States. It combined radiofrequency and ultrasound to address both skin tightening and fat, reinforcing the idea that multi-layer treatment would define the future of non-surgical aesthetics.

The CoolSculpting Phenomenon—and Its Problems

In 2016, the industry experienced one of its biggest marketing explosions with fat-freezing technology, most notably CoolSculpting. The concept was elegant: kill fat cells by freezing them. The company backed that idea with an estimated $100 million marketing budget in a single year, rivaling legacy consumer brands. Suddenly, even my male friends in Tennessee—far outside the traditional aesthetics demographic—were seeing billboards and knew the term. Clinically, however, the reality was far more complicated.

When fat cells are destroyed via freezing, the body doesn’t automatically retract surrounding tissue. The result can be loose skin and a deflated appearance. Worse, a rare but real complication—paradoxical adipose hyperplasia—occurs in roughly 1 in 100 patients, causing fat to grow back harder, denser, and larger, often requiring surgical removal.

Despite the millions in revenue it generated for those who offered it, it was a hard pass for us. Public perception shifted after a high-profile case involving Linda Evangelista, reinforcing a lesson this industry repeatedly learns: marketing momentum can outrun clinical reality.

2018: Muscle Stimulation Goes Mainstream

In 2018, another category exploded with the introduction of Emsculpt, a muscle-stimulation platform. Fueled by social media, professional athletes, fitness influencers, and pop stars openly showcased six-pack abs achieved without traditional workouts. For the first time, non-surgical aesthetics wasn’t just about appearance—it was about strength and function, including measurable benefits like core engagement and back support. Treatment volumes and social media impressions were staggering, pulling an entirely new audience into the industry.

2020–2025: Microneedling Meets RF—and Its Dark Side

Around 2020, another surge followed with devices combining radiofrequency and microneedling. Adoption accelerated rapidly and peaked around 2025. But delivering RF energy below the skin’s surface—particularly in unskilled hands—introduced real risks, including scarring and tissue damage, prompting recent FDA warnings.

The Core Tech Stack

Despite the constant stream of “revolutionary” devices, the underlying technology hasn’t fundamentally changed. It still comes down to a few energy categories under different brand names:

•  Radiofrequency for deep tissue heating and skin tightening

•  Muscle stimulation using electromagnetic energy

•  Acoustic wave therapy, adapted from physical therapy, effective for cellulite

•  Ultrasound, useful on the body but riskier near facial nerves

•  Lasers, primarily superficial

Most new platforms are variations on these themes, marketed as breakthroughs despite incremental differences.

Business Realities: Bottles, Bills, and the One-Off Model

Injectables operate on tight margins. At $300–$600 per unit, providers must choose between being generous and eroding profit, or conservative and risking unhappy patients. Unlike traditional B2B businesses, aesthetics has no contracts. Every day starts at zero. Patients are in and out within an hour, and if no one walks in tomorrow, revenue stops tomorrow. Even surgery doesn’t solve this. Facelifts costing $30,000 to $250,000 don’t stop aging. Collagen and muscle decline roughly 2% per year after age 40, meaning even the most expensive surgery fades with time.

Manufacturers as the Hidden Competitors

One of the biggest challenges in aesthetics isn’t other practices—it’s the manufacturers. Devices costing hundreds of thousands of dollars are quickly followed by “upgrades” promising marginal gains. Direct-to-consumer marketing trains patients to demand the latest version, forcing providers onto a technology treadmill. Many devices now include planned obsolescence—limited pulse counts, mandatory service contracts, or pay-to-play models that create a permanent cost per treatment for the practice.

The Bottom Line

Aesthetics sits at the intersection of medicine, consumer desire, and marketing hype. Clinical innovation has been incremental, not revolutionary, despite what advertising suggests. Friends regularly send me the “next big thing.” It’s almost always a variation on energy sources discovered years ago. The winners in this business aren’t defined by who owns the newest machine, but by judgment, restraint, experience, and business fundamentals.

Old truths still apply. If a tree falls in the forest and no one sees it, it doesn’t matter how advanced the technology is. With devices capable of generating $1,000–$4,000 an hour, many assume aesthetics is an easy entry. It isn’t. Unlike B2B, you can’t put people under contract. The real challenge—now more than ever—is building a predictable, sustainable stream of clients. And no machine can do that for you.

1st Do no Harm. Non Surgical Cosmetics

I will always talk about the truth. Society is way too obsessed with ridiculous made-up standards of beauty. It also changes as often as the styles do. Was anyone alive to witness Twiggy? She was a famous model of the seventies. Google her. What is everyone going to do with their inflated butts when the style goes back to thin?

How about “heroin chic.” That was the ’90s’. Skinny-worship was only twenty years ago. Remember Calvin Klein ads, Kate Moss? You can bet your big fat ass that it will be back. So first, get some self-confidence.

BBL or Brazilian butt lift is the deadliest procedure in plastic surgery. I made this easy for you. What a horror show. There is way more horrible information on this, then I anticipated. PS you can do emsculpt on your butt and increase the muscle in your butt for slight lift. Its only 15%. My point is not to do that, Its simply to avoid the most dangerous plastic surgery and not die. Why dont you just go to the gym? You will look and feel better overall and you might actually meet a like minded person. Unless you are obsessed with spending a ton of money to meet a superficial jerk. Think about that one?

So you are spending a boat load of money to attract a superficial jerk. Why not just put a want ad that says I want a superficial jerk? Oh right you are doing that. It is as plain as the unusually large butt that is following you around. It will probably attract an ass to follow you. It is literally a signal to what type of partner you are looking for.

Regarding other minor non-surgical procedures, not BBL. To be clear if you are in your forties to sixties and have discretionary income and getting rid of loose skin, cellulite, stretch marks or another simple procedure would make YOU happy, have fun. Do not come in and say you are doing it for someone else. A good doctor will turn you away. You can tighten skin, make collagen and even tighten the muscles on your face. I would argue, in a sense it is all natural. As you work your way down your body you can do the same to any body part. IF You have the discretionary income and it bothers you. I would also propose Emsculpt on your abs makes sense because it is your core, it protects and in some cases helps people with back issues. Its not a medical treatment but many claim it has helped their back.

I loved getting the first six pack in my life, ironically in my sixties. This is me 2018. This was with Emsculpt. Emsculpt Neo is the one with the heat that is slightly more effective. I was pretty happy with my abs after emsculpt.

So I have done Emsculpt Neo for my abs and my face with Thermage and Emface and Exilis.

In the video, I repeated my admonition then I explained all the procedures to avoid. I then explained what I believe after 17 years experience answering probably 17,000 phone calls (3 a day) and traveling the world researching technologies both in the US and in Europe, the best non surgical technologies.

When I went to Monte Carlo in 2009, there was only one other New York doctor there. There was only lasers and the first non surgical device for skin tightening, Thermage. This industry did not exist. In fact the reason we went, was Dr Lisa was lecturing on the new Thermage face tip called the CPT. There was no Coolsculpting. That was 2016 and it is a disaster, still the most dangerous treatment. There was no Emsculpt. That was 2018.

So all of the geniuses that you have been speaking to or taking advice from, even Doctors, have no clue about all of the technologies. In fact any time you hear a doctor speaking about a technology, its not even that they believe in it or know anything about it. They are being paid by the manufacturer to read a slide show prepared by the manufacturer. They dont even know what they are talking about.

The Monte Carlo Anti Aging Congress, accepts studies provided by doctors about technologies that were just released that year. The study is made by the doctor. Thermage didnt even know Dr Lisa had submitted the study.

So that is real research, not a doctor being paid to narrate a slide show.

Again to be clear, If you have discretionary income and it would make you happy to see a few less wrinkles, a bit more collagen and tighter muscles, less sag on your face when you look in the mirror I am all for it, and I do it. Want a tone tummy? Great. I know women absolutely love getting rid of stratch marks even thirty years later ( Carboxytherapy works on any color stretch mark on any color skin at any age). Cellulite is also a bummer. People love getting rid of that. Its all good. I am advocating against making young ladies and girls feel bad about looking normal. If you are under thirty go to the gym, lose weight your skin will bounce back.

PS I probably havent moved to florida yet because it scares me. All the women look alien, with inflated lips and butts and boobs. There are natural approaches to all of this. Even Botox, a good doctor will never freeze everything. They will soften the lines. They will not make you look like a deer in the headlights, if you really want Botox. Although, check out my forehead no Botox for probably 20 years, Just emface and thermage, making collagen and tightening muscles, not freezing them.

Ill end with this story told by Wayne Dyer. A 40 year old woman goes to her doctor for her annual physical. She comes home all excited and tells her husband, ” the doctor told me I have the breasts of a 20 year old, the legs of a thirty year old, and the face of a twenty year old” her husband replies, “What did he say about your forty year old ass?” ” “Oh she replies your name didnt come up”. Thats what I am talking about. Do what makes you happy go to the gym, work out at home. Always happy to help even if its just a pep talk.

To Be Clear  This is all about the cheeks and the jaw line   The lower face.  That is the area that the Thermage treatment  has the most obvious and dramatic results. That was more than 30 years ago 1988, I did not have great genes.  Notice the loss of volume and collagen in the cheeks.  Pronounced nasolabial  folds  ( the lines from the nose to the corner of the mouth).  Obvious thin skin, skin is practically hanging    Loss of volume in upper cheeks   This was so long ago I even had hair and a black leather jacket LOL.  I am not even smiling and you can see the skin is loose.

Age 53, 18 years later or 12 years ago.   I had one Thermage treatment.  Actually an improvement over my 35 year old self.  You can see my skin is tighter.   But I lost a lot if not all of  the collagen in my cheeks.  I think these pictures really make a compelling argument for the loss of collagen and a saggy lower jaw as the primary signs of aging,  as well as the restoration of collagen with Thermage as the most natural solution for a youthful face.   It is far superior to a face lift     Who needs thin skin stretched out over bones ? That doesn’t make you look younger. It merely makes you look older or like an old person with a facelift

ABOVE  AGE 64   I had one Thermage treatment September 2017  I was afraid I was finally getting too old and could no longer fight some loose skin, But its all gone and tight again.  Now if I can just get some botox for my forehead I can look even younger LOL

BELOW AGE  65  THERMAGE Today at 65  A perfectly straight jaw line, No double chin, No Chin fat, No jowls, No loose skin.  No Nasolabial folds or marionette lines (from the nose to the corner of the mouth.  Those lines are almost invisible which I actually had at 35 ( the top photo)

ABOVE  65   First time I did a second treatment in less than a year ( like the ladies do)   You have to admit, me at 64 and 65 there is zero signs of aging in my lower face. Do I need botox ? Yes.  Could I use a full head of hair ? LOL  maybe.  But as far as what Thermage can do.  Its done !  Go back and look at me at 35.  Would really expect me to have a perfectly straight jaw line, no jowls, no double chin.  Not a drop of loose skin on the lower face 30 years later ?  PS I  did not get cheek implants.  In fact its obvious that I havent even had botox in over 2 years  Any filler or botox is completely worn off.  This is Thermage and only Thermage
