Thermage Before and After

Thermage before and after treatment, stomach or abdomen.

This is a perfect example of a thermage before and after. thermage is a great treatment for someone who had a baby or recently lost weight and has loose skin.  Because the treatment affects the collagen layer.  There is an immediate tightening. The collagen layer also holds back the fat ( by the way thats the first clue to understanding cellulite, it has everything to do with the collagen mesh that holds back fat but thats in another post)  So besides the immediate tightening long term your body is stimulated to grow more collagen which further tightens and smooths either your face or your body,  More thermage before and afters here

The Comprehensive Guide to Cellulite

Understanding, Preventing, and Treating Dimpling Skin

1. What Is Cellulite?

Cellulite, also known as gynoid lipodystrophy or dermopanniculosis deformans, refers to a dimpled, “orange peel” appearance of skin—typically affecting the thighs, buttocks, and abdomen. It arises not from excess fat or toxins, but from structural changes beneath the skin: fat lobules push upward while fibrous septae pull downward, resulting in uneven surface texture healthline.com+15en.wikipedia.org+15dovepress.com+15pmc.ncbi.nlm.nih.gov+1en.wikipedia.org+1.

It affects 80–98% of women post-puberty, regardless of body size, due to gender-specific skin and connective tissue architecture researchgate.net+2pmc.ncbi.nlm.nih.gov+2pmc.ncbi.nlm.nih.gov+2.

2. How It Forms: The Science Behind the Dimples

Cellulite involves a complex interplay of anatomical and physiological factors:

  1. Subcutaneous Fat & Septae Structure: Women’s fibrous septae run perpendicularly to the skin and are fewer, which allows fat lobules to protrude more readily onlinelibrary.wiley.com+15pmc.ncbi.nlm.nih.gov+15pmc.ncbi.nlm.nih.gov+15.
  2. Hormonal Influence: Estrogen, insulin, catecholamines, thyroid hormones, and prolactin influence fat accumulation, collagen support, and circulation—all contributing to cellulite .
  3. Genetic Predisposition: Specific genetic variations (like ACE and HIF1A polymorphisms) are linked to differential susceptibility pmc.ncbi.nlm.nih.gov+2en.wikipedia.org+2pmc.ncbi.nlm.nih.gov+2.
  4. Microcirculation & Inflammation: Impaired blood flow, lymph drainage, and subtle inflammation contribute to fat deposition under dermal layers byrdie.com+9en.wikipedia.org+9lipotherapeia.com+9.
  5. Aging & Lifestyle: Declining collagen and elastin, combined with sedentary behavior, poor diet, and dehydration, exacerbate the condition .

3. Preventive Lifestyle Strategies

While genetics and gender play a primary role, several lifestyle practices can significantly reduce the appearance and progression of cellulite:

A. Hydration

Well-hydrated skin maintains elasticity and helps minimize visible dimpling. Studies suggest better skin texture and reduced water retention with sufficient daily fluid intake verywellhealth.com+1artemis.co+1.

B. Balanced, Anti-Inflammatory Nutrition

A diet abundant in fruits, vegetables, whole grains, omega-3 fats, and lean proteins supports collagen synthesis and microcirculation. Limit processed sugars, excess salt, and refined carbohydrates verywellhealth.com.

C. Targeted Exercise

D. Skin Care & Bodywork

4. Natural & At‑Home Remedies: Realistic Results

Dry Brushing

Enhances exfoliation and blood flow. May temporarily affect skin texture, but blemishes return without maintenance .

Manual or Mechanical Massage

Lymphatic and mechanical massage (like Endermologie) can improve microcirculation. FDA-approved devices exist, but results tend to dissipate within weeks unless treatments continue mayoclinic.org+1healthline.com+1.

Topical Agents

Caffeine and retinol-based creams offer temporary tightening by promoting circulation or epidermal thickening—but don’t address deeper structural causes .

5. Medical & Device-Based Therapies: What Actually Works

A. Acoustic Wave Therapy (AWT)

  • Mechanism: Uses rapid acoustic pulses to shearingly disrupt fibrous septae, enhance microcirculation, and trigger collagen remodeling.
  • Evidence:
  • Conclusion: AWT is one of the most robust non-invasive options with lasting improvements.

B. Radio‑Frequency (RF) Skin Tightening

  • Mechanism: RF heats deep dermal layers to stimulate neocollagenesis, elastogenesis, and dermal thickening—tightening the skin pmc.ncbi.nlm.nih.gov.
  • Clinical Use: Multiple bipolar, tripolar, and microneedle devices are FDA-cleared for skin laxity and cellulite reduction en.wikipedia.org.

C. Combined AWT + RF

Integrating AWT with RF attacks cellulite on both fronts:

  1. AWT mechanically disrupts fibrous bands.
  2. RF tightens and thickens the skin.
  3. Combined effect yields smoother, firmer results with better longevity than either alone.

D. Subcision & Laser-based Procedures

E. Why Most Other Treatments (e.g., CoolSculpting, Ultrasound-only, Injections) Fail

  • Fat reduction devices like CoolSculpting don’t target septae and can make cellulite more visible by thinning surrounding tissue.
  • Low‑intensity ultrasound lacks septae disruption capacity en.wikipedia.org+1arxiv.org+1.
  • Injectables (e.g., collagenase) or liposuction are invasive, with unpredictable outcomes and higher complication rates .

6. Lymphatic Massage: Minor Yet Supportive

Manual or mechanical lymphatic drainage can temporarily reduce tissue fluid and improve skin tone—but doesn’t substantially remodel structural causes. A trial found only 33% of participants had a cellulite grade improvement, with no significant body composition changes pmc.ncbi.nlm.nih.gov+1healthline.com+1. The massage aids as a complementary adjunct—not a stand-alone solution.

7. The Best-in-Class Protocol: Evidence-Based, Multi‑Modal Strategy

Step 1: Acoustic Wave Therapy — one or two sessions spaced weeks apart to fracture septae.

Step 2: Radio‑Frequency Skin Tightening — multiple sessions over several weeks to boost skin thickness and firmness.

Step 3: Supportive Lymphatic Therapy — manual or mechanical massage between treatments to reduce swelling and enhance circulation.

Step 4: Lifestyle Integration

  • Drink ~0.5 oz water per pound of bodyweight daily.
  • Prioritize anti-inflammatory, collagen-boosting foods.
  • Engage in cardio + strength training 3–4 times weekly.
  • Add dry brushing and massage as maintenance tools at home.

Step 5: Maintenance
After initial treatment, schedule RF/AWT booster sessions every 6–12 months, alongside consistent lifestyle practices.

8. Why Our Approach Stands Above the Rest

Most standalone treatments lack robust evidence or long-term results. By combining AWT’s structural impact with RF’s tightening power, supported by lymphatic and lifestyle enhancements, this integrated method delivers superior and sustained outcomes—backed by high-quality clinical trials.

9. Appendices

A. Key Clinical References

B. Terminology Glossary

  • Fibrous septae: connective tissue bands anchoring skin to underlying fascia.
  • Neocollagenesis: formation of new collagen within dermal layers.
  • Subcision: surgical cut of fibrous septae under anesthesia.

Final Takeaway

Cellulite is a normal condition reflecting underlying skin and tissue biomechanics. While minor improvements can be achieved via lifestyle choices and manual treatments, substantial, lasting change requires targeting the root causes. The gold standard approach combines:

  • Acoustic Wave Therapy to disrupt structural septae,
  • Radio-Frequency to rebuild skin quality,
  • Lymphatic support and healthy habits to preserve results.

When scientifically grounded, this integrated method is both safe and effective—and far more powerful than single-modality or unproven alternatives.

How Not to Choose your Cosmetic Doctor

Maybe a few screen shots can help you understand, “First do no harm”. The proliferation of Med Spas are scary. Why not start with a doctor’s office that has been around for at least 15 years? I am not saying you are going to die, but even that is not out of the question. PS at Precision Aesthetics we have performed at least 20 procedures a week or. 20,000 procedures, where everyone was thrilled with their results.

PS Maybe one out of a thousand left a review. People just dont want to jump through those hoops. Plus now Google wants your real name and all kinds of information about you. So nobody is running to say, “I had a ton of cellulite, stretch marks and a double chin and they made it all go away”. You think that is going to happen? PS the rules are really strict, you are not even allowed to offer an incentive for a review. So when you see hundreds of reviews you are probably looking at the next company that is going to be sued for millions of dollars because they didnt follow the rules.

Finally dont think that laser hair removal is a no brainer. In New York there are places where anyone is operating these devices. Yet it is a class 4 laser or the strongest one. It is so easy to burn someone. I have had a bunch of treatments. Its a total catch 22 if its not high enough the root of the hair is not destroyed and the hair grows back. If its too strong you can scar someone. I dont even know what to advise in that arena, we dont even offer it.

What about Emsculpt?

EMSCULPT is quickly becoming one of the most popular and sought-after body sculpting treatments available, with its cutting-edge technology and proven results.

EMSCULPT is a state-of-the-art, non-invasive body sculpting treatment that uses high-intensity focused electromagnetic (HIFEM) technology to tone and strengthen muscles. It is designed to target specific muscle groups in the abs and buttocks, helping to sculpt and shape the body for a more toned appearance.

The treatment works by using a device that delivers strong electromagnetic energy to the targeted muscles. This energy causes the muscle fibers to contract and relax, simulating the effects of intense exercise. Over the course of several treatments, this process helps to build and tone the muscles, resulting in improved definition and a more sculpted appearance.

One of the main benefits of EMSCULPT is that it is non-surgical and non-invasive, so there is no downtime required after a session. Patients are able to return to their normal activities immediately. In comparison to traditional body sculpting treatments such as liposuction, EMSCULPT provides a safe and effective alternative with minimal risk and no recovery time.

It is important to get treated at a doctor’s office to determine if it is right for you and to develop a personalized treatment plan. In fact I was recently told by our rep from their company that even at a Doctor’s office they treated a person with a pacemaker. Given the nature of HIFEM electromagnetic pulses they could have given the person a heart attack. Please limit your searched to Doctor’s offices with at least 15 years of experience for your own safety let alone to get the best results.

In fact there are so many options to get the best emsculpt treatment, whether its emsculpt emsculpt neo, emsculpt flex let me direct you to each option and what it targets. Options for your best emsculpt treatment.

Getting Started Non Surgical Treatments Explained

I have been answering phone calls for more than 10 years. There are some who call who have already decided what treatment they want and merely want a price. They ask for a procedure by name. Ironically those are the stupidest. Seriously unless you are a doctor in this particular industry with at least 15 year experience treating thousands of people each year, how would you know which technology is going to get the best results for someone with combination of varying amounts of loose skin, fat plus or minus some cellulite or stretch marks ? We all lose collagen every day of our life, so if you are over 30 years old you probably have some loose skin, or maybe just want to prevent the signs of aging. Sorry I digress could not resist.

So this is for those who are starting out. I get a lot of phone calls from people who have no clue what all these strange names and technologies are all about, Thermage, Exilis, Ultherapy, Ultrashape, Velashape, Coolsculpting, Emsculpt, Emtone, Emsculpt. I could list 100 more, but you get the idea. This all began in 2002. Thermage was the first radiofrequency device approved by the FDA in 2002 to tighten skin and treat wrinkles and since that time over 2 million treatments have been performed. So for you newbies Let me explain this with an analogy we can all relate to. Most of us have some idea that lasers have been around for a long time. You can “burn” off sun spots and other superficial blemishes. What is different here is radio frequency. We are all also familiar with a microwave oven. ( PS the doctor hates when I make this analogy) But learning is usually best accomplished by analogies. You know how your microwave can heat the middle of a chicken without browning the skin ? Well thats the beauty of radio frequency. if you tried to use a laser to reduce even a very fine wrinkle you would burn the skin before you approached the depth to which you would need to get to improve the wrinkle. So what Thermage did was have that radio frequency in a hand held wand so the technician could go over your face ( and body) and “hit” the collagen for an immediate tightening and subsequently because of the stimulation of the collagen you grow more collagen over the next 6 months. Think how elegant. Instead of losing collagen every day you are growing more collagen. Hello Gwyneth Paltrow, we have your secret.

Are you being sold a bill of goods ?

I wrote something this morning for Precision Aesthetics. I then recorded it as a podcast. But let me be a little redundant here because sometimes you need to hear something said a number of different ways before it makes sense. Especially if its about something you know nothing about. Also I sometimes take for granted all of the non surgical technologies that are in the beauty industry and think the average person has heard of them. So a little bit of background about it all then maybe what I recorded might make more sense. Let me just mention a few technologies that we offer. Thermage tightens skin, Exilis melts fat, Emsculpt make muscles by forcing 20,000 contractions, Carboxytherapy reduces stretch marks. When we see a new client we design a program that is personalized to address their particular needs. Everyone has their own unique combinations of loose skin, stretch marks, cellulite, that would require some combination of technologies to get the best results for that individual. So someone “selling” you their only machine makes absolutely no sense whatsoever. See if this makes any sense with that context

Six Pack Abs ( my answer on quora)

That is me. Nobody could have been more surprised than I was. I never had abs. Even when I was thirty years younger running 5 miles a day and 10 pounds lighter. It was after six treatments over the course of three months. Not according to their protocol, which was 4 treatments more quickly. PS I took a collage photo of what I was eating. I am not proud of it. But I did not want to alter my diet. There is nothing stupider than someone dieting and claiming a treatment helped them. For example the HCG protocol, which is injecting yourself daily with a hormone and being on a 1500 calorie a day diet. Did everyone just get stupid? 1500 calories a day leaves the average man at approximately 1500 calories a day deficit. Over the course of one month he would lose around 15 pounds. So I purposely continued to eat as poorly as I was eating winter 2018. Plus I answer the phones and I need to be able to sleep at night and know that I was full on integrity when I explained how great my results were. In fact I called one of the top study doctors in Texas to ask him why I got such great results. Seemingly better than anyone else. I said it makes no sense. You say the best results are one week later so perhaps the fact that I stretched out my treatments to every other week benefited me. He said they had tried a number of protocols. Personally I believe if you want to sell a $250,000 machine to a doctor you better be telling them they are going to earn $4000 every 10 days vs $3200 after three months I continued to eat as poorly as I was eating that winter. I even took photos of the chinese food, french fries, ice cream and all the other nasty things I was eating that winter. But my results were astonishing.

I also write a lot of answers on the website Quora on skin tightening, stretch marks, loose skin, cellulite and the newest Emsculpt for Abs, I am going to start putting some of those answers here because I speak to a lot of people who love to do a lot of research before they commit to spending a lot of money and choosing who should do their treatments ( as they should) So here is my first one I just wrote today.. Well actually its my one hundredth, but its the first one I am copying here.

How do I know if my abs are not showing because they are not developed enough or your body fat percentage is too high to see them? John Aslanian, http://www.PrecisionAestheticsmd.com NY Research International & USAnswered 1h ago

Because we offer Emsculpt we do know from their FDA approval some approximate numbers. Emsculpt is a machine that induces your muscles to do crunches. Its not your $19 ab belt. Its a $200,000 machine that forces 20,000 crunches in 30 minutes. When they got their FDA approval they presented research with 4 treatments and MRI and ultrasound before and after. They showed a 16% increase in muscle and 19% reduction in fat. However only people with less than 25% body fat showed any visual improvement, let alone a six pack. Personally I got great results at 22% body fat. Here are my photos. Ironically when i was in my thirties i was 12% body fat and did not have a six pack. So here are my personal before and after Emsculpt photos after four and after six treatments. So for reference I think you can conclude you do need around 100,000 crunches and I imagine I probably would have had even better visible results if my body fat was in the low teens vs 22%.
